Improving Health through Community Leadership, Collaboration, and Culture

The Greater Humboldt Park Community of Wellness is a broad coalition of organizations and individuals dedicated to improving our community’s health status through community leadership, collaboration and culture.  Our membership includes community members and over thirty organizations, including healthcare providers, human service agencies, advocacy groups, research institutions, schools, and institutions of higher education.

Mission and Vision

Our MISSION is to facilitate a comprehensive vision of wellness by working collaboratively with a broad array of people and organizations to leverage their complementary strengths and capabilities to solve community health problems.

Our VISION is to create a Community of Wellness in Greater Humboldt Park, where health is an aspect of all community development.  Residents will be informed about and have ready access to culturally sensitive and affordable healthcare and services, and can take a pro-active role in their family’s health.
