The Greater Humboldt Park Community of Wellness is a broad coalition of organizations and individuals dedicated to improving our community's health status through community leadership, collaboration and culture. Our membership includes community members and over fifty organizations, including healthcare providers, human service agencies, advocacy groups, research institutions, schools, and institutions of higher education.
We define health based on the World Health Organization Declaration of Alma-Ata, International Conference on Primary Health Care, 1978:
“Health, which is a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity, is a fundamental human right and that the attainment of the highest possible level of health is a most important world-wide social goal whose realization requires the action of many other social and economic sectors in addition to the health sector.”
The Community of Wellness is composed of organization and individual membership, Health Priorities, Projects and a Steering Committee. Health Priorities are organized around 8 prioritized disparities. Health Prioritieseducate themselves and the public at large coordinate services and relationships, advocate publicly, andintervene. Interventions are multi-organizational Projects aimed at changing the actual health status or career-readiness of large numbers of people. The Steering Committee works with the Community of Wellness’ Director to ensure strategic direction, coordination and monitoring of progress on the Community of Wellness Plan.
